Welcome to Feather River Hospital. Be assured you will be provided with the highest quality and state-of-the-art care. Whether you will be here as an inpatient or outpatient, the registration process is the same.
Before arriving, remember to gather all paperwork given to you by your physician. At the time of registration, you will need to provide the following information:
Full name and address
Social Security number
Name of an emergency contact
Insurance cards
Name and address of policy holder on the insurance card if different than patient
Name of employer
Name and address of where to send the bill if the procedure is for liability or workers' compensation
Registration is located in the main lobby next to the the information desk. Once you enter the registration area, a friendly clerk will guide you through the process and assist you in filling out any necessary paperwork.
