Recovering at Home

Your physician will decide when you are ready to be discharged.  When you are ready to go, a nurse will assist you to gather you belongings.  Wheelchair transportation is available to your car.  A responsible adult needs to be available to provide transportation for you.

A discharge planner will, if needed, assist you and you family in making arrangements for care after your stay at the hospital.  The Case Manager can help in selecting a convalescent facility, arranging for home health care services, or locating durable medical equipment.  A Social Worker can help identify support services.
