It is your responsibility to understand the terms and conditions of your insurance coverage. All patients are encouraged to call their insurance companies to obtain accurate information about individual benefits and any pre-certification or authorizations needed for services.

Your insurer may require written or telephonic pre-certification or pre-authorization from you or your physician. Receiving appropriate authorization does not guarantee payment if you have already used your benefit level within the timeframe specified by your insurance company, or if the service is not a covered benefit within your plan.
