Whether you are using inpatient or outpatient services, the information you to the admitting clerk is the same.  When registering, please have your address, personal identification (Driver's License, State Issued ID Card, etc.), social security number, emergency contact, insurance cards, name of employer, and all special insurance billing information.

Please leave valuables at home before you arrive at the hospital. If you do need an item stored, ask your nurse about placing it in the hospital safe.  An itemized receipt and a claim ticket will be provided for you.
