Feather River Hospital's Social Workers and RN Case Managers can help. These health care professionals are specially trained to help relieve your concerns by facilitating solutions for financial, physical, and emotional needs during this stressful time.

While you're in the hospital, Social Workers and Case Managers provide a variety of assistance. They help patients and family members cope with the crisis surrounding a medical emergency, relieve anxiety about the financial aspects of health care, and assist in dealing with the trauma of a terminal illness.

How do I arrange for home health and other home care services?

Our Social Workers and RN Case Managers are available to assist in making referrals (as ordered by your physician) to the agency of your choice.

What if a patient requires admission to a skilled nursing facility following discharge from Feather River Hospital?

Feather River Hospital's Case Managers maintain close contact with local skilled nursing facilities. Should you require skilled nursing care after your discharge, our RN Case Managers will coordinate the transfer to the facility of your choice.

What if I need rehabilitative services?

If cardiac, physical, speech or occupational rehabilitation may be helpful, the Case Manager will coordinate rehabilitative services as ordered by your physician.

How will Medicare's new Diagnostic Related Groupings (DRG's) work and how will the DRG system affect patients?

With constant revisions in Medicare and MediCal reimbursement levels, it's more important than ever that patients and their families work closely with our Case Managers. Feather River Hospital's Case Managers can explain how this system may effect you.
